Fertility Counselling

We assess the risk factors pertaining to the couple under evaluation by history, examination and specific tests and proceed with a two way discussion regarding the best suitable treatment options for them, success rates for each procedure, expected time to pregnancy and average costs of treatment, which help the couple to take the best informed decision. We also provide a lifestyle counselling specific to the couple including diet to enhance fertility, exercise, habit changes and methods of relaxation.

Ovulation induction

It is considered as the first line of treatment for couples with problems related to ovulation. The per cycle pregnancy rates and number of cycles offered varies depending on the age of the female, semen analysis of the husband and the presence of other contributory factors. If ovulatory dysfunction is the sole identified cause, six to nine cycles of ovulation induction is generally advised for a women less than 35 year.

Intrauterine insemination (IUI)

Usually accompanied with stimulating the ovaries by giving tablets and injections, timing the ovulation by giving the injections for follicular rupture (trigger). Husband’s sample is collected after 36 hrs of giving the trigger injection, which is processed in the lab to get a concentrated sample of motile sperms. The sample is injected into wife’s uterus with a catheter. This is a simple OPD based procedure with a success rate of 15 -18% per cycle in the best prognosis couple. Usually suggested for couples with unexplained infertility, mild issues with the semen parameters, minimal and mild endometriosis, those with coital issues and suspected immunological causes. IUI is beneficial maximum up to 4 cycles

Invitro Fertilisation (IVF) and Intra Cytoplasmic Sperm Injection (ICSI)

ICSI is the most advanced mode of fertility treatment which includes pre IVF counselling and optimisation of patient factors, stimulation of ovaries by giving hormonal injections, collecting the eggs 36 hrs after giving the injection for maturation, forming embryos by injecting single sperm into the egg, culturing the embryos for 3- 5 days in the incubator, transferring the resulting embryos into the uterus in the same cycle or freezing in the liquid nitrogen AST-196c and transferring in the coming cycles. Per cycle pregnancy rate can vary from 10-70 % depending on many factors.

M TESE

The most advanced form of surgical sperm retrieval which is really useful for those with absence of sperm in the ejaculate due to very low sperm producing potential. The tubules where sperm harbours is identified by using micro surgical instruments and 25-30 times magnification and sperm is extracted from the tubules and ICSI is done. The chance of getting sperm by M TESE varies from 10 – 70% depending on the causes.

Fertility Enhancing Laparoscopic Services
